DevonThink is doing a fine job updating indexed files, but uniquely not files inside any of my Obsidian vaults, which are inside my ~/Documents/OBSIDIAN folder.
I have to delete the indexed group inside DevonThink and then CMD-OPT-drag my vault folder back again into my big INDEXED group (inside DevonThink).
Why would files modified by Obsidian be different from ones modified by other apps?
Is there anything in the trash when it fails?
Are you using Obsidian Sync?

No. But just today I found out that if I quit DT and then do Update Indexed Items two or three times that 60 seconds later, the missing items sometimes show up.
But I just found out that this is happening in other folders as well (which I posted as a new question).

That sounds like you could be experiencing problems because of iCloud’s notorious sloth and unreliability. (I’m assuming you are synchronising the Documents folder using iCloud.)
Update Indexed Items only works when the indexed folder of concern is either (1) the selected item, or (2) the “enclosing group of the current group”. It’s not a global command.
