Take a printed list of items, scan and export to a database?

I don’t know how well the title explains what I want to do.

I have a long list of routes that I need to learn for delivery.

It’s typewritten on paper.

It’s pages long, with columns depicting the house number range, street name, route ID#

I want to somehow be able to scan this information on my scanner and then be able to digitize the information in such a way that it can be imported into a flash card program (such as Mental Case) that I can study from.

So far, I have scanned the documents using Devonthink Pro. How can I convert them to a database with specific item columns in a format that can then be imported by another software?
