Yes, groups vs. databases is an important distinction.

A quick search on the forum for custom database icon:

June 2020:

Customized icons on Databases and Favorites are not synced

The customized icons are not part of the database, it’s just a Finder icon and therefore currently not synchronized. And there are no plans to change this as e.g. in a team people might prefer different icons.

Welcome @EmmCeeSq The custom icons on groups in a DEVONthink database sync between Macs. Nicely, the custom group icons also sync to DEVONthink To Go Custom icons on databases do not.

Database icons are actually not part of the database and therefore not synchronized, they’re just custom icons of the database package (.dtBase2 or .dtSparse) in the filesystem. These icons are also used by the Finder and can be changed via its Info window.

I doubt this has changed.
