Some of my items have a grey icon in the name field that looks like an old fashioned speech balloon (located in the same area as the duplicate icon). I’ve no clue what it means!

As I recall, that indicates that there’s content in the Spotlight Comments field of the document’s Info panel.
In DT’s built-in help, look Documentation > Windows and Panels > Main Window > Items. Related topic:
