In case of rich text it’s definitely an issue of macOS’ text engine. The Markdown editor uses the same text engine plus some rich text formatting, maybe that’s triggering it in rare cases?
I encountered a similar problem
When the text in the Markdown file exceeded the window display range and the scroll bar appeared, and you turn on Syntax highlighting, you will jump to the top of the file when editing.
It’s the same whether it‘s in the editing view or in side-by-side mode.
The indentation of the list is abnormal too. When there is no text after the list symbol and I want to indent, the back cannot be achieved.
turn off Syntax highlighting seems ok.
Which version of macOS and DEVONthink do you use? The scrollbar settings (see System Settings > Appearance) might matter too.

It doesn’t seem to appear the issue after selecting “always”

Just today, I had the same issue and can confirm that from all the approaches discussed in this thread, only the “Syntax highlighting” setting in DT’s settings → Files → Markdown has an influence on the problem.
I am observing this on macOS Sequoia, DT Pro 4.1.1. The document I am editing is a Markdown document that I created from the sorter. In terms of Markdown features, it only contains headings, links and bulleted lists. In order for the problem to appear, you need to create more text than fits into the edit window. Some lorem ipsum is sufficient.
My DT settings:
System-wide, my Scrollbars are set to “always” be visible.
The jumping effect is independent of other settings, including fonts and display settings from the “Editing” → “Format” dialog. Also, only changing “WYSIWYG-images and links” while keeping “Syntax Highlighting” active does not remedy the issue.
Typewriter-like scrolling does not affect the issue.
I can also observe the problem when scrolling into the middle of longer Markdown texts and type there – the scrollbar of the document visibly jumps up and down.
Could you share a document that is affected by this issue? A screenshot of Settings > Editing > Format would be useful too.

This is already sufficient. Once I reach this line and type in it, the jumping effect is observed.

I have the same issue and can confirm that, also in my setup (macOS 15.7.4, DT 4.2.1), it appears to be related to syntax highlighting (disabling it is the only action that seems to have any effect on the “jumping” behaviour)
